Startup Request

The pre-startup checklist must be transmitted along with the panel schedules and start-up request form to schedule a start-up date. By providing a general overview of the site preparations you can reduce the risk for additional charges due to the site not being ready for factory commissioning. Any additional charges incurred at the time of commissioning due to the site not being ready, inaccurate or incomplete paperwork, or incomplete wiring will be the responsibility of the requesting party per our Lighting Services quote policy.

General An electrician familiar with the installation will be available with ladders / lifting equipment (if required) All the equipment is wired and powered. All equipment shown on the Bill of Materials in your submittal has been received and installed. Any special training/safety classes required to access site. Tech be required to complete a background check. Please note any special security measures (surrender of IDs, security clearances, escorts, training classes, etc...) required to access the site and /

Lighting Control Panels All low voltage switches have been installed, wires run and terminated at the proper inputs. All equipment shown is wired to the proper loads. If using a photocell, the photosensor must be installed, wires run and terminated at the proper input (Interior and Exterior). TERMS & CONDITIONS

3 WEEK NOTICE REQUIRED

Commissioning Requests must be received three weeks prior to the time Cristal is to arrive on site. The 3 week lead time is necessary to review all system documentation, ensure that documentation is complete and accurate, obtain any missing information, as well as program the system, prior to arriving on-site. Cristal will not schedule on-site visits, until all information below is verified as being complete. Please contact your Cristal Project Coordinator to help you complete these forms if you have ANY questions.

CANCELLATION & RESCHEDULING POLICY

If a Commissioning Request Form has been submitted and confirmed, and appropriate travel arrangements have been made (i.e. flight, train, car rental, hotel, etc.) requestee will be held responsible for all associated cancellation fees. Cristal will invoice purchasee all associated cancellation fees. Failure of back-charge payment will void all system warranty. In the event you cancel your confirmed date without reasonable notice, you may be charged to reschedule.

LATE SCHEDULE NOTICE POLICY

Cristal Controls understands that some jobs may require an expedited start-up time frame. For start-up on projects that require start-up notice less than the standard twenty-one (21) business day lead time, the customer may request expedited service for a fast-track fee. Cristal Controls will attempt to accommodate an expedited service request, but it remains strictly subject to availability and may not be possible based on current service demand. Payment of the expedited service fee will be required prior to scheduling the on-site start up.
